Step 1
|
Carefully read through the directions in the portal to determine whether you need
to fill out an
exempt, expedited, or full review application. Explanations are provided under each category as to what conditions must be present for your research.

Step 2
|
Complete the application in the portal. You may save a copy and come back later,
however the application must be printed out and signed for submission. To
print your application, use the print button found in the application toolbar.

Step 3
|
Append the necessary paper attachments
and mail all of your materials including the printed and signed application to the
address found on the main page.

If you are having trouble filling out your application, feel free to
email the System Administrator for help.
You may also download the application in Microsoft Word format
and complete them manually. The same rules regarding signatures and attachments
apply to these applications as well.
